Practitioner 以下NP)とクリニカル・ナース・スペシャリスト(Clinical Nurse Specialist 以下 CNS)が中心となってコンサルタントする脳卒中のケア実践の構造，プロセス，アウトカムについて知見を深めた。\n脳卒中は救命された後も，高次脳機能障害が併発することから，退院後を見越したケアの提供が必要となる。米国では，いかに早期受診をさせるかという[brain attack]の概念が早くから導入され，血栓溶解療法等が先駆的に行われ，比較的迅速な対応がなされている。\n今回，脳卒中専門看護師の専門性，機能，役割，教育システムを明確にすることを研究目的として，\n米国テキサス大学ヒューストン校メディカルセンターとメリーランド大学メディカルセンターの脳\n卒中ケア・ユニット(Stroke Care Unit 以下 SCU)での体制化での脳卒中のケアを行う NP と CNS に聞き取り調査を行い脳卒中専門看護師の育成にかかる要件を分析した。その結果，脳卒中急性期におけるケアのさまざまなシステムの中で医師や他の職種と共に協働し，中心となってコンサルテーションしていく NP・CNS の機能と役割について抽出されたことは，脳卒中という緊急を要する疾患の初療から退院までの一連の過程において，「迅速」「的確」「積極性」というキーワードが抽出された。\nわが国では，治療における診断技術や検査のオーダーなどの診療部門は医師に限定されている，そうした法的な問題もあるが，脳卒中患者に直接関わることは米国も日本も変わりがない。NP・CNS
