| 内容記述 | 高齢社会にあるわが国では,高齢者に対するケアの問題は保健医療において重要な課題である。しかし,医療の現場では,ケアの提供者である医師と受給者である患者との間が必ずしも良好な関係にあるとは言えない。その原因の一つに,高齢者に対するケアのあり方や重視するケアの中味について,両者に間に意識差があるのではないかと考えた。そこで,高齢者ケアと高齢者を支える制度である介護保険制度について,医師と患者を対象に意識調査を実施した。その結果,患者は医師よりも高齢者ケアを重視していた。特に,リハビリテーションや日常生活援助に関することについて,患者,とりわけ女性は,大多数は重視する回答をした。医師は患者ほど高齢者ケアのそれぞれの項目を重視しておらず,そういった医師,患者間の意識差が日常の診療過程に齟齬を生んでいると考えられる。反対に介護保険制度については,医師の方が患者よりも積極的に同制度を評価していた。医師も患者も年齢が高くなるほど,また,女性より男性の方が介護保険を有益であると評価していた。また患者の過半数は,審査の不透明性や地域格差などを同制度の問題にしていたが,審査する側である医師は問題としていなかった。介護保険制度が有効に機能しているかどうかは今後,この制度が根付く過程で,再度,検討する必要がある。(英文抄録：Health care for aged people has been a highly important issue since Japan has became an aged society. However, there is not necessarily in reliable relations between physicians and patients, and their families. A major reason of the above phenomenon is believed to be the discrepancies in opinions for geriatric care between both groups. We investigated how physicians and patients regard the geriatric care categorically and the newly-introduced system of care insurance. The data indicated that patients group took the geriatric care more seriously than physicians did. Patients, especially female, considered skill-items related to rehabilitation and daily living-care more important. Such discrepancies in opinions for "care" may cause some troubles in both medical and care services. On the contrary, the system of care insurance was more positively accepted by physicians, by elder people than by young people, and by men than by women, respectively. The majority of patients regarded the not-open process of decision making of care provision and regional gaps in services as problematic in the system. The feasiblity and effectiveness of the system of care insurance have to be carefully examined through the daily works.) | |||||||||
